InnovAge Holding Corp. is a leading healthcare delivery platform that provides all-inclusive, capitated care to high-cost seniors, many of whom are dual-eligible. The company's programs aim to improve the quality of care for participants, enable them to remain in their homes safely for as long as possible, and reduce the use of expensive care settings like hospitals and nursing homes. Through its Program of All-Inclusive Care for the Elderly ("PACE"), InnovAge offers a wide range of medical and ancillary services. These include in-home care services (skilled, unskilled, and personal care), in-center services such as primary care, physical therapy, occupational therapy, speech therapy, dental services, mental health and psychiatric services, meals, and activities. InnovAge also provides transportation to and from PACE centers and third-party medical appointments, as well as care management. The company contracts directly with government payors like Medicare and Medicaid, without relying on third-party administrative organizations or health plans. InnovAge Holding Corp. and certain wholly owned subsidiaries were formed as for-profit corporations on May 13, 2016, to acquire Total Community Options, Inc. d/b/a InnovAge, which was established in May 2007. Total Community Options, Inc. and some of its subsidiaries converted from not-for-profit to for-profit corporations during this purchase. In March 2021, coinciding with its initial public offering ("IPO"), the company changed its name from TCO Group Holdings, Inc. to InnovAge Holding Corp.
